This patient had a known diagnosis of lymphoma at the time this mammogram was taken. If we had not known this, then a breast primary with nodal involvement would have been possible, but we do not see any abnormality on the mammogram that might be a primary tumor. The nodes seen are markedly abnormal, the fatty hila almost completely effaced. This finding would be very unusual for a benign reactive node.
OK this is a tricky question. The BI-RADS system was not developed to deal with malignancies other than breast cancer. The classifications, such as “Suspicious” designate the likelihood of breast cancer, not other tumors. Thus a finding such as this lymph node, which now has a normal appearance, is considered benign in that we have no suspicion of breast cancer. A surgeon would have to remove the node to determine whether there is any residual lymphomatous involvement
